A senior engineer knows why the retry logic behaves the way it does. Extracting that costs them an hour they do not have, produces notes nobody can use, and has to happen again in six months when the behaviour changes.
So it does not happen. The knowledge stays where it is, and the documentation describes the parts that were easy to describe.

Sessions designed to get a specialist to explain what they know in the order a person actually needs it, not the order they happen to remember it. The structure does the work, so the session takes an hour rather than a morning.

What comes out is already in the shape our standards require, which is why it can move to published content quickly and consistently across a distributed team.
Maintained state
Captured knowledge is tracked against the thing it describes. When the product moves, we know what is now wrong, rather than discovering it when a customer does.
